Animal Surrender Requests

If you are contacting us to surrender an animal, please fill out the Animal Surrender Form below. One form per animal is required.

Please note that submission of this form is NOT an agreement that Faithful Friends Pet Rescue & Rehoming will accept the animal for rehoming. All requests are evaluated to determine our ability to rehome the animal with a suitable home that meets the animal's specific needs.
